Clinical Pharmacy Services

Hudson Headwaters’ clinical pharmacists are part of the extended care team who work directly with our patients to help reduce medication-related problems. Clinical pharmacy services are available for primary care patients at all our health center locations, either in-person or virtually.

Our clinical pharmacists provide comprehensive medication management (CMM), an in-depth review of all your medications (prescription and non-prescription) to determine whether each of them is appropriate, effective, safe and able to be taken as intended.

Clinical pharmacists also review all medication changes with you after you are discharged from the hospital. Talk to your primary care provider for a referral to our clinical pharmacy services.
